They are called Peepers (little known fact - they were invented by puppeteer Hobey Ford). There is a chance that Puppets For Goodness Sake in Stoney Creek might have some, otherwise check out http://www.peeperspuppet.com.
The movement in a lot of the cable controlled puppets I see in commercials and TV shows is often very stiff and mechanical, but the movement looks very natural in that video, nice work!
Depends what kind of patterns you are looking for. You'll find tons of links by searching the forum for "puppet pattern", but to get you started:

These are by Hansjürgen Fettig. I suspect they are from "Hand and Rod Puppets" which was published around 1973, but it could be one of his other books.
If the monitors don't have to be portable and a 4:3 aspect ratio (standard TV screen, instead of widescreen) is OK I highly recommend Commodore Amiga or Commodore 64 monitors if you can track them down. Typically they cost $20 or less, are full colour, take standard video input and are...
I forgot to mention that I find 3/4" foam ideal to work with for most things, but it's hard to find in some areas. Half inch is good too if you can find it dense enough (stiff enough that it keeps it's shape), but one inch will work too.

There isn't a body or hand pattern at the moment (I keep meaning to make some up to go with it) but...
As SesameKermie said, there are a lot of old thread about eyelids here on the forum, but this article on the Puppet Building Wiki should help get you started.
One thing that I have noticed is very important to pay attention to the angle you the foam at. Unless you are deliberately wanting to make a bevel cut or something like that be sure that you cut the foam with a blade at a 90 degree angle from the foam.
Band saws work really well for cutting...
L200 should be fine in theory, but it really depends on a variety of factors. I use sheets that are approximately 1/2" thick all the time for puppet bodies without any problems, although I do reinforce it with dress maker's boning.
I think the whole fiasco with those blinking LED ads for Aqua Teen Hunger Force in Boston a week or so ago was just stupidity of the highest level. The authorities don't seem to be able to tell the difference between a bomb and a simple LED light (not encouraging if you live in Boston and rely...
