It's never been commercially released (to my knowledge) and if it's ever been aired on TV it was probably prior to VCRs becoming widespread. I'd assume there are bootlegs floating around somewhere if you look hard enough.

Off the top of my head, I would try cutting different triangle shapes and try folding or bending them and then gluing the edges down to the mouthplate.

Usually if you design a character you own the copyright. If the builder does than they own it unless they agree otherwise.
Normally if you want the copyright and didn't design the puppet you have to pay what's called a "buy out" fee for the rights. The amount of the fee is usually negotiated...

If it's a generic "person puppet" you can probably purchase a pattern through a one of several companies. Search the forum for "puppet building pattern" and you should find threads with all the necessary info.
Well, "Diva" is a generic term. You could probably get away with it (although you're better off with your own original name) as long as the puppets don't look like Steve's.
