I don't think Kevin Clash worked on Follow That Bird, though I could be wrong. I think the first Muppet thing he worked on up here in Toronto was the Muppets' 25th Anniversary special. Also on "Follow That Bird" a lot of the Canadian Muppeteers were subbing on various Muppets.
I don't know where you would find that exact pattern - since the show was made in the UK over 25 years ago I doubt it's commercially available and it was probably a custom job by a scenic painter anyway. You should be able to search the net and find out how to do a faux gold leaf finish. I think...

Read over the first part of the tutorial Buckshot mentioned. For a head like Zoot's you probably need 6 or 7 wedges that are four times as long as they are wide. It's hard to say exactly, most puppet building requires a certain amount of trial and error.
