I also learned of a series of three segments called "By the Numbers" that were shown in May 1984 - the final weeks of Season 15. Each of the segments aired just once, and were never seen on first-run episodes again afterwards.

In an almost-reversal of how some segments had their last two showings years apart, there's a New York remote segment called Pop Ons, first seen in episode 1315, a season 10 episode. It only aired three times - first in season 10, then once in season 17, and its final airing in season 18.

There have been other segments where the second airing came years after the first and then had very few other airings. Biff and Sully in the snow premiered in a season 7 episode and only aired again during the season 13 snow arc, and there's the Forgetful Jones segment Cowboy Cole which only aired twice, once in season 18 and again in season 22.

There's probably more that only aired about twice or so that had such long gaps in airing.
