Yep; in one sketch he pulls the neck of his shirt down, and we can see his orange skin, so apparently the orange body (used for scenes in the bathtub) is always with the puppet.
Nope; those other CG characters were controlled like puppets, similar to Waldo C. Graphic. The book and the other animals in it were made from a computer, but no puppetry work (except for the dubbed voices). Well, I guess it'd be more animation than puppetry, more like "Toy Story" animation...

The earliest I've seen him referred to by name is in a 1981 book, "The SS Circus of Opposites" (which also contains what may be the earliest illustrations of the Honkers, and even Pearl and Deena appear).
Either that or he's VERY busy with other projects. After all, Jim Henson has always been credited to the Muppets' success (just like how Phil is usually noted as the webmaster of MC), but there's thousands of people who have made it what it is. That's what makes it great; it's a group effort...
