I know in the old days, the first episode of any series is automatically dubbed as "The Pilot Episode". In my day, "The Pilot Episode" is a short/proto-type episode that is separate from the actual series.

Sometimes, it still works like that. "Seinfeld"'s pilot- "The Seinfeld Chronicles" bears much resemblance to the final series, and was good enough to be put into syndication with the rest of the show.

Yeah, it varies on what the network actually wants. Sometimes they go back and just reshoot some (but not all) scenes in the original pilot that eventually airs.
