It is from one of the street scenes from an episode in the 80s or early 90s when everybody either has a wound or is imagining they have one. I believe it is a closing scene. The imagination scene contains Elmo, Telly Monster, and various humans imagining what they could not have done if their thumb was in a cast. The lines may be slightly exaggerated and/or out of order.
The last two lines of the poem are recited outside the imagination scene. I don't remember the seventh line, but the last one is "we (all) didn't hurt our thumbs!"If I hurt my thumb, I couldn't use my silverware.
If I hurt my thumb, I couldn't throw a ball.
If I hurt my thumb, I couldn't tie my shoelaces,
And I couldn't button any buttons at all.
If I hurt my thumb, I couldn't use a pencil to write you a letter.
I couldn't use my drumsticks to play my drums.
