1) Already answered, from 1972 (episode #406) to 1976.
2) I don't think a particular character made him, but maybe Don Sahlin, Kermit Love, and Caroly Wilcox were in charge. In his first appearance, he says he's come to Mulberry Street (which he's confused...

That's a guess, too. I've been watching the Muppets for a long time, so I kind of get the idea of how certain Muppeteers move the puppets; Jerry often skips syllables, Richard uses double takes, etc. They're subtle, but if you're use to the performers doing the voice and puppet at the same...

I never knew about this until I got the clip. It wasn't until a bit later that they re-used the fur to make an entirely new puppet.
I'm guessing, but Grover's voice during the scene doesn't sound like any of the other principle performers, so I'm assuming that Kathy did it. If you compare the voice to, say, Mokey Fraggle, or Leona (from the first two seasons of "Between the Lions), you can hear the similarities.
Yes, the three monsters are different. The three in the flute sketch are Anything Monsters, while the ones in the phone booth sketch are AMs dressed to look like monsters.
I think Kathy Mullen played Grover in the Hooper's Store scene. She may have also played other characters when Jim, Frank, or Jerry weren't available (hence her name being listed among the primary performers). Richard Hunt also played the Count in a majority of his scenes, with dubbed lines...
Well, I can't tell. There were four other monsters in the sketch: Herry, a short lavendar one with frizzy purple hair and red eyes, a furry lavendar one with horns and a red nose, and the green bird monster. Jerry played Herry and the short monster, but the only mouth I see moving when the name...
