The Season One episodes are set out *more* like a typical comedy-variety series a la Ed Sullivan, whereas from Season Two onwards the guests began to take a more active part in the backstage mayhem. Not sure who we can thank for that - Jerry Juhl maybe?

I think there's a bit of confusion here - as I understand it characters from other companies are able to appear in the specials without any problems. It's the fact that they can't appear on the cover of a commercial release of this special.
The only people going "bonkers" are those (such as yourself) who are criticising Disney unnecessarily. DVDs usually don't get formally announced until at least a month until they are due to be released. Disney don't want to make the mistake of announcing a title too early, otherwise they could...
Ummm.... we are talking about three rather obscure Muppet specials from the 1960's; somehow I don't think they would be big sellers. Besides that, Disney may not even own the rights to the specials.
Get a Grip folks! TMS is not the only thing that Disney will be releasing on DVD this year. Please keep that in mind. I'm sure that there will be an announcement in due course, but as I've said time and again - You just have to be patient and wait. And if you can't, then that is your bad luck.
That's all true, but I'm interested in finding out whether there were specially made ones for the UK airings. Other ITC produced shows had them, and my hypothesis is that TMS did have some.
No, those were the special "trailers" that were recorded for each episode. I'm talking about ad caps that appeared before (and possibly after) the commercials.

They didn't; remember the Muppets didn't always play "themselves" as per their TMS personas in the films. As for when Sweetums' personality changed, check the episode guide for season one of TMS in this site.

Apart from the head shot of Dave Goelz, look at the back left of the screen towards the end of the number as the glee club move off and you can see the left arm and shoulder of one of the performers.
Isn't it interesting that they kept in these types of mistakes in the first season of TMS but...
