Making warts/bumps/moles etc?

Okay so this may be a silly question... if so, apologies! But I'm looking to create a very bumpy, warty puppet and I'm stumped on how to build these. On all the muppets I've seen with things like this (Constantine, Angel, etc) they appear so seamless and perfectly shaped. I'm wondering how that look might be achieved reliably...

My current thoughts are:
1. They are sewn partially down and then stuffed with a tiny bit of something to give them the raised look.

2. They're sculpted on the underfoam first & then the fleece is glued down around them.

3. The wart fabric is pulled around a bead or something similar, glued, and then glued down to the fleece.
