Anytime you have facial animation, heads have to be at least partly hollow. Bert's is, and re-enforced to the eyebrow mechanics.

There's a great often-used photo of Jim actually working on the original Bert head, and you can see (kind of) how it's built.

No idea about patterns-sorry!
